GL & Personal Accident Insurance : Claims Requirements
In case of a claim, the Policyholder shall send an intimation through a written notice within thirty days of the occurrence
of any of the Events giving rise to a Benefit. DNI will process the claim once all claim documents are received
The Policyholder shall provide the claim documents in respect of the following:
I. In Case of Death Claims :
- An official letter signed by the Policyholder intimating the claim
- An official letter signed by the Policyholder intimating the claim
- Duly completed claim form
- Original Death Certificate
- Passport Copy of deceased with valid visa page at the time of death.
- Visa Page Cancellation Copy
- Labour Card Copy
- Wherever legally possible, a Postmortem Report will be required, along with a Police Report if Death was due to an accident.
- A detailed Medical Report is to be submitted if the actual cause of Death is not clearly mentioned on the Death Certificate.
- 3 months salary slips prior to the date of death.
- Other documents to support the claim

If death has not occurred in UAE, the following additional documents would be required:
The original Death Certificate must be submitted duly notarized and attested by the UAE Embassy in the country where death took place. Subsequently, the same should be attested by the Embassy of the country in the UAE and Ministry of Foreign Affairs in UAE.
II. In Case of Permanent Total and/or Partial Disability Claims :
- An official letter signed by the Policyholder intimating the claim
- Duly completed claim form
- Original full and complete medical report showing diagnosis, future prognosis and specifying any percentage of disability from the treating doctor or hospital/clinic.
- Original discharge summary if in-hospitalization was involved from the treating doctor or hospital/clinic.
- Original copies of results of any diagnostic tests.
- Original Sick leave certificate
- Original Medical/Hospital Bills
- Original Pharmacy Bills
- Prescription Slips
- 3-month salary slips
- Passport Copy and visa page copy
- Labour Card Copy
- Any other documents pertaining to the claim which the company may require.
III. In Case of Temporary Total Disability Claims:
- An official letter signed by the Policyholder intimating the claim
- Duly completed claim form
- Original full and complete medical report showing diagnosis, future prognosis and specifying any percentage of disability from the treating doctor or hospital/clinic.
- Original discharge summary if in-hospitalization was involve from the treating doctor or hospital/clinic.
- Original copies of results of any diagnostic tests.
- Sick Leave Certificate from the treating doctor.
- 3-month salary slips
- Passport Copy and visa page copy
- Labour Card Copy
- Any other documents pertaining to the claim which the company may require.
IV. In Case of Medical Expenses Claims :
- An official letter signed by the Policyholder intimating the claim
- Duly completed claim form
- Original full and complete medical report showing diagnosis, future prognosis and specifying any percentage of disability from the treating doctor or hospital/clinic.
- Original discharge summary if in-hospitalization was involve from the treating doctor or hospital/clinic.
- Original copies of results of any diagnostic tests.
- Original bills pertaining to the claim.
- Prescription to support all pharmacy bills.
- 3-month salary slips
- Passport Copy and visa page copy
- Labour Card Copy
- Any other documents pertaining to the claim which the company may require.
V. In Case of Repatriation Expenses Claims :
In addition to the requirements for the Death/Disability Claims the following will be required.
- Airway bill & payment receipt of freight charges.
- Receipts for embalming charges / collection of body from the morgue / coffin charges etc.
- Copy of ticket jacket of person accompanying the body.
- Receipt for cost of the ticket issued for accompanying person
- Any other charges pertaining to the repatriation of the body to native or home country.
